My sister is applying for a grant for her medical practice in Georgia. She has applied for this grant and the next step is to have people VOTE for her.
Could you please be so kind as to go to the site below, add her information and then click on the VOTE button?
Thanks so very much!

please go to www.missionsmallbusiness.com go to the bottom and enter in Campbell Family Medicine, Suwanee, GA and then vote yes for her.

If you wish to read the last draft of the form she had to submit, read on.

Tell us about your business; how successful is it and why is it unique?
Our country is facing its worst health crisis ever. We have beaten most acute illness, but chronic disease is encroaching on our population at younger and younger ages. Our modern health care system is not healthy and not caring. It is designed beautifully to manage acute health crisis, but fails miserably at the management of chronic disease. We have evolved a care system so cumbersome that it allows time for only two things: prescriptions or surgery. Most of what makes people sick cannot be corrected with a pill! We can the future of primary care, and are making an impact one life at a time. Using integrative and alternative medicine and the evidence based guidelines of functional medicine, we get to the root cause of many of our patients' symptoms. Food sensitivities, hormone imbalances, metal toxicity, and genetically determined defects in metabolism can be detected, treated and restored to a more physiologic balance. In a fee-for-care model, we use nutrition coaching, supplements, detoxification, Bio-identical Hormone Replacement Therapy, vitamins (especially Vitamin D!), and much more to help patients lose weight, normalize lipid and blood sugar levels, come off of maintenance medications and improve cognitive performance and energy levels. We coach people on a path to wellness and the stories they share of health recoveries are remarkable and inspiring. Patients are begging for well-trained doctors who can help them to bridge the gap between the integrative therapies they already use (because they work: herbal therapies, acupuncture, herbs, vitamins, massage, colonics etc) AND the best of modern medicine. Not enough doctors are versed in the language, interactions, strengths and weaknesses of traditional vs. modern treatments. We currently have an 8 month wait list for new clients, and are working hard to find a sustainable way to reach more people.

How is your business involved with the community you serve?
We teach a lifestyle of better health choices and nutritional wellness to our community. We offer free education lectures on a monthly to quarterly basis. We use our Facebook page to share health information of and by our community. Dr Campbell is the mother of three teenage daughters, an integral part of the community, an active member of the Chamber of Commerce, and a volunteer for Habitat for Humanity.
More importantly, our business model was created to serve the underprivileged by integrating a tithing policy. Five percent of the business cash profits are donated to charity. Five percent of Dr. Campbell's services are provided to clients at no monetary fee. However, coming from a background of indigent healthcare, she learned early-on that free is often valueless, so her services are not free. Instead, her "scholarship patients" obtain their care by completing a log sheet of volunteer hours, or their families do the volunteering if the patient is disabled. Fifteen hours of completed community service earns 3 months of unlimited doctor's visits. In this way, the low income patients earn their care with dignity, the community gets served, and Dr. Campbell can quantify her charity work without overwhelming the system. When we hold open houses, we use raffle proceed to fund an account to pay for nutritional supplements for our scholarship patients, making the full spectrum of care we offer available to the neediest of those we serve.

What would a $250k grant mean to your business plan and how will you utilize the funds to ensure long-term growth and stability?
Earning $250,000 in grant funding would accelerate our business growth forward by 5-10 years.
We would leverage the funds for seed money to start or accelerate four basic areas of the business.
First, we would pay off the startup debt. Being in business for 8 years, we are making payments on some of initial business startup costs and subsequent equipment purchases. We expanded in 2010, and took on more debt. We have never been late on a payment, but it would be AMAZING to be debt free!
Second, we would purchase 2 pieces of new equipment to serve more patient needs.
Third, we would hire, train and certify additional individual lifestyle coaches to assist patients in making the changes recommended by the clinical staff. The money would be seed funds to start and implement the training, but the ongoing costs would be paid for by the patients utilizing the services.
Lastly, we would use the money to partner with a social media firm to revamp, refine and update our websites and social networking. We are only accessing a fraction of our target audience and need assistance in reaching those we would like to serve. The larger the number of paying clients, the more scholarships we can offer to those without means.

What types of challenges can you identify with your plan and how will you overcome them?
We are already achieving much of what we set out to do, but on a smaller scale than we dream for. In our care model, each physician has a maximum number of patients. As such, we are stalled in our growth, as we do not have enough uniquely trained physician staff. We will be moving to a larger space in 2 months, and have offered a job to an associate physician to start in September 2012. By then, we hope to be staffed to handle some of our growth, and to give care to those on our waiting list, but we need a better marketing campaign. By having our debt paid off, a larger staff, and a targeted marketing program, we can serve a larger patient base, and have the infrastructure to reach and teach more adults and children how achieve their own potential without the burden chronic disease symptoms.

Describe the talent on your team and how they make your business successful.
Our business is successful because of our care team, and because of our successful clinical outcomes. Until now, we have had a zero dollar marketing budget and grew by word of mouth alone to a full practice and a long waiting list. Dr. Campbell has been board certified in family medicine for 20 years, but she recently also took boards in Holistic and Integrative Medicine. She lectures internationally on subjects of Vitamin D and wellness. She is not afraid to "think outside the box"' and tells patients that she has the "biggest black bag in town" with innumerable tools to achieve better health. In 2010, she started a networking group "The Consortium of Integrative Care Practitioners of Atlanta" (www.theCICPA.com) so that integrative care practitioners would have a resource to find the BEST integrative care for themselves and their clients. . She is a brilliant, vibrant and personable star in our local medical community and wants to reach more people. Our care team involves a multidisciplinary staff whose sole purpose is to help clients achieve greater wellness. Our staff includes lifestyle educators, a reiki master, a reflexologist, a breast thermographer, lifestyle and core dynamics coaches, and EFT practitioner. We offer massage, far-infra-red sauna, IV nutrition support, and detoxification counseling. We have several decades of cumulative experience and countless cases of lives changed with our support and advice.

Additional relevant information you would like to share.
We are creating a template for a small business success. While successful in its own right, Campbell Family Medicine wants to expand this model to reach more people and dreams to expand so that the doctor's office is only one small business in an entire MALL of wellness services. Using the success of CFM as a template model, we will embark on a larger project: designing Atlanta’s first environmentally friendly medical mall dedicated to accessing comprehensive out-patient alternative and integrative health care, disease prevention and anti-aging services. This mall’s goal is to create a new paradigm for patient care centers that would establish a trademark as THE preeminent provider of integrative care services designed specifically for the prevention, treatment and REVERSAL of chronic disease, as well as optimizing the aging process through a combination of multi-disciplinary on-site health care practices.

This facility (and eventually, others like it) will combine state-of-the-art medical imaging and diagnostics with conventional and integrative healthcare. It is the mission of the wellness mall to promote maximum health and wellness, prevention, and health education to the community BY the community.
The center is designed with four general areas of focus. It will feature diagnostic services, a therapy area, a practitioner section, and retail stores. In addition, there will be 300 seat auditorium with a state of the art “Smart Board”, presentation system, and demonstration kitchen. The mall will promote wellness in body, mind, and spirit with leasable space and common areas that display a prominent water feature and a space for meditation and prayer.
It will be built as a certified green building project by The Leadership in Energy and Environmental Design (LEED) Green Building Rating System™. It will be constructed with recycled and renewable building materials, will utilize geo-thermal and solar energy sources, will have xeriscape landscaping, gray water re-use, and will be connected to the community by walking paths and public transportation. We want to attract a compounding pharmacy and vitamin/nutraceutical retailer, organic grocery store, natural cosmetics store, and a bookstore. Utilizing BioIdentical Hormone Replacement therapy, nutritional therapies, detoxification programs, energy therapies and other lifestyle changes, practitioners can design a unique care plan by accessing services available in a single location that should result in significant, positive and objective impact on patient’s ultimate health outcomes, reverse chronic disease, and support healthy disease free aging for everyone in our community.
